Transitions is an innovative program new to Orleans County. It is designed for people with a life-limiting illness who can benefit from supportive assistance as they deal with end-of-life issues.
Transitions offers compassionate support, assistance, resource linkage, and enhanced volunteer services to individuals and their families who:
- Have a life limiting illness with a relative prognosis of one year or less
- Are undergoing curative treatment for their disease
- Are not yet ready to make a decision about entering a hospice program
- Need assistance in accessing community and medical resources to manage their disease and/or the challenges it brings
The Transitions Coordinator lends help by providing:
- A listening ear
- Information and linkage to area community services
- Home visits and phone check-in calls
- A connection with a trained volunteer
The Transitions Volunteer may provide:
- Companionship
- Emotional support
- Help with errands and shopping
- Respite for caregivers
Transitions is available at no charge to any resident of Orleans County, New York who has been diagnosed with a life-limiting illness and may or may not be undergoing curative treatment. It's available wherever the client resides; in their own home, or assisted living center.
When you or someone you love are diagnosed with a life-limiting illness, it affects your life in innumerable ways. It can be confusing to know where to turn for help with all the changes a serious illness brings. It is not unusual to feel hopeful one minute and discouraged the next.
