What is the Transitions Program?

Transitions was designed to help those patients and their families who have just begun to face the realities of a life-limiting illness.

For many people this is an overwhelming stage of their life, and it can be confusing to know where to turn for help with all the changes and decisions that these realities bring about.

Transitions is a free program provided by Hospice of Orleans to help you through this difficult time. Our Transitions team consists of a specially assigned coordinator and volunteer who offer enhanced levels of compassionate support, practical advice, and clinical care.
